New patients
Everything you need before your first visit: your sleep questionnaire, what to bring, and what happens at your first exam.
Your sleep questionnaire
A short questionnaire about your sleep, daytime energy and health helps us see where to start. Your body mass index (BMI) is part of it, and you can work it out below before you come in.
BMI calculator
The calculation happens on your device. Nothing you enter is sent anywhere.
Forms
Filling out a form before you arrive saves time at your visit.
- Epworth Sleepiness Scale (PDF, 16 KB): a one-page daytime sleepiness questionnaire you can fill out before your visit and bring with you
- You can complete the rest of your new patient forms at the office.
What to bring
- Photo ID
- Medical insurance card and Medicare card
- A list of your medications
- Results from any past sleep study
- Your CPAP details, if you have tried CPAP
Your first visit
Your first visit is an extensive, paid consultation with Dr. Lynch. We share as much as we can up front, so nothing about it is a surprise.
- Your story. How you sleep, how you feel during the day, and what you have tried.
- X-rays, if needed. Dr. Lynch decides which X-rays you need.
- Mouth and neck exam. A check of the soft tissues in your mouth and an oral cancer screening, including feeling the lymph nodes along your jaw and neck.
- Jaw joint exam. You open and close while Dr. Lynch checks your jaw joints. If you have jaw pain, she also checks your head, neck and upper back.
- Teeth and gums. Tooth wear patterns and gum health, which can point to grinding and sleep apnea.
- Airway. Your jaw, tongue, tonsils and bite.
- Your results and options. Dr. Lynch reviews what she found, explains your options, and answers your questions.
